Scheduling appointments
To schedule an appointment please call us at 435-634-0358. Our office staff have been trained to handle most routine telephone calls. The staff may occasionally have to check with Dr. Chase before returning calls or answering some questions. On your first visit you will be asked to fill out a form with general medical questions.
